All women interested in participating in Flagstaff Women’s Soccer League 2012: Please follow the instructions so we may get you registered ASAP!!

1. You must be 18 years old by June 10 to officially play.
2. All players MUST wear shin guards during games.
3. There will be NO SLIDE TACKLING at any time. This is a multi-skill level recreational league. We want to have fun and be able to walk the next morning!

1. Print out AWSL Registration Form.
2. Print out the (Flagstaff Women’s Soccer League) FWSL 2012 Input Form.
3. Complete BOTH forms. (If you are already playing in the Phoenix league (Monsoons or Altitudes), then you do NOT have to fill out the application form.)
4. Attach a COLOR copy of your driver’s license.   It must be LEGIBLE. This serves as verification of your age for the league, and the picture will be used for your player identification card.  (If you are currently on the Monsoons or Altitudes, you do NOT have to submit a copy of your driver’s license.)
5. Please mail these forms and color copy of your driver’s license, along with a check for $55 made out to Flagstaff Women’s Soccer League to:
